Documents Required for a Motor Insurance Claim

Private Vehicles

• Duly filled and signed claim form — download here
• Copy of RC. If vehicle is less than 3 months old and RC is unavailable — tax receipt and purchase invoice.
• Copy of driving licence of the person driving at the time of accident
• Copy of insurance policy
• Copy of PAN card or Form 60
• Copy of Aadhaar card
• FIR or police report (depending on loss type)
• PUC certificate (if applicable)

Commercial Vehicles

All of the above, plus:

• Copy of Fitness Certificate
• Copy of Route Permit and Load Challan (not applicable for Small Commercial Vehicles)
